Overview
Chronic Kidney Disease (CKD) multidisciplinary clinics are now established at Bundaberg Base Hospital and Hervey Bay Hospital. The clinics include health professionals such as a Dietician, Nurse Practitioner/Clinical Nurse Consultant, Psychologist (Hervey Bay), Diabetic Educator (Bundaberg), Social worker and Pharmacist (Hervey Bay).
The aim of the clinic is to slow and prevent progression of kidney disease through the provision of monitoring, education and support for the patients. Currently the patient population is referred from the Nephrology clinic but there are plans for this clinic to work in partnership with several General Practices who are interested in this model of care.
The CKD Management Resource Kit was distributed in late 2007 by GP Links and this has received positive feedback.
The CKD resource kit contains guidelines and a referral form, if followed and completed, it will assist in correctly prioritising patients.

In Hervey Bay a CKD Weight loss group has been established for patients who are keen to lose weight and get fit. This involves one supervised gym session / week at the gym at Hervey Bay Hospital and then two days of walking in Hervey Bay (approx 20 – 30 minutes) per week. All exercise activities are supervised by qualified health professionals. Please contact the renal unit (41206850) for more information and assistance.
