Overview:
Allied Health Centre: Diabetes Education Clinic
From January 2008 GP Links has provided to general practitioners the service of a Credentialed Diabetes Educator to deliver diabetes education to their clients.
The service operates from the Mater Hospital Professional Centre and can be accessed by referrals under the Medicare Enhanced Primary Care (EPC) Program.
All clients need to have a GPMP & TCA in place with Medicare and to be eligible for an initial assessment to determine their suitability for either group education or an individualized education and self management program.
Whether clients are newly diagnosed, or have had diabetes for a number of years, any education will benefit their long term outcome.
Assessment for group education can be used to deliver self care information and the EPC Medicare referral can be used to build on this initial information.
All Medicare request forms should be included with the referrals indicating the number of visits for your clients.
To ensure sustainability of the Service:
• The credentialed diabetic educator is registered for MBS payments
• Patient’s co-payments will be implemented for service provision.
• DVA Gold Card and some White Card holders will not be required to pay the co-payment.
