Overview:
Janelle Anderson from GP Links is currently the only licensed provider on the Wide Bay/Fraser Coast for the “Reset Your Life” diabetes prevention program designed by Baker IDI Heart &
Diabetes Institute in collaboration with the Australian General Practice Network. This
program is recognised by the Department of Health and Ageing as an accredited Lifestyle
Modification Program (LMP).
Patient Eligibility
The program is available through referral from a GP. Patients are eligible for the subsidised program if they:
- Are aged 40 - 49 years (or 15 - 54 years and identify as Aboriginal Torres Strait Islander); and
- Have been assessed as being at 'high risk' of developing Type 2 Diabetes (ie a score of >12 on AUSDRISK; and
- Diagnosis of Type 2 Diabetes Mellitus has been excluded; and
- Have provided consent for patient information to be passed on to the Divisions of General Practice network, as indicated on the GP referral form for program monitoring and evaluation purposes; and
- Have not previously been subsidised for an LMP
Making referrals
Fax the referral form and a copy of the AUSDRISK tool to GP Links Wide Bay 4151 0794.
As participants may be involved in exercise during the sessions, we ask that you provide a list of
current medications and a list of any medical conditions that may impact on their ability to
participate.
Program Availability
Group size is limited to 15 participants. Programs are run from the GP Links conference room or convenient community locations. It is our aim to provide services across the entire Wide Bay/Fraser Coast. GP Links recognizes that many participants may be working therefore the initial programs will be offered after hours.
Cost of the Program
If the participant is eligible for the subsidised program, there is a one off $50 co-payment to cover administration costs. There is no charge for pension and concession card holders (co-payment subsidised, please indicate concession status on GP referral form). Participants paying the one off $50 co-payment may be eligible for private health fund rebates. Potential participants who are ineligible for the subsidised program can pay the private fee of $300 to do the program. Please note that the Division will provide general practices with updated information on the availability of LMP providers as more programs become accredited.

These MBS items enable GPs to review patients aged 40 to 49 years (inclusive) who are at high risk of developing type 2 diabetes and instigate early interventions to assist in the prevention of type 2 diabetes. They also apply to ATSI patients from 15 to 54 years of age.
“Reset Your Life” Program Schedule and Content
The “Reset Your Life” lifestyle modification program is run over 6 months. The schedule for the program sessions is as follows:
1. Introduction and what is pre-diabetes?
2. Physical activity and healthy weight
3. Introduction to healthy eating/carbohydrates & fibre
4. Preventing cardiovascular disease
5. Staying motivated and positive thinking
6. Stress Management
