Program aims
1. To undertake strategic development of systems and processes to improve patient care particularly in relation to the transition of patients between primary, secondary and tertiary care settings.
2. To bring a GP perspective to policy and planning for the provision of health care in the Wide Bay Health Service, Northern Cluster catchment.

Program governance
The GPHLP Advisory Group provides advice and support to the GP-Hospital Liaison Program on all matters relating to the program’s aims. It currently meets monthly.
Membership of the advisory group is drawn from Bundaberg Health Service executive, the GP Links Wide Bay CEO, GPHLO and the program’s Project Officer.

Program objectives
To bring a general practice perspective to the policy & planning processes of the acute sector.
Through establishment of relationships with key hospital planning & policy personnel & participation in key hospital committees, the liaison team are able to advocate on behalf of general practice to provide enhanced patient management between the hospital and general practice settings.
To actively promote the engagement of the general practice community with the local health service and other primary care stakeholders.
Promoting open lines of communication between general practice, hospitals and other identified stakeholders facilitates enhanced and timely management of patient care.
To facilitate improvements in transfer of care across the whole healthcare continuum.
Through liaison between healthcare stakeholders, pathways of care are mapped and improvements facilitated where required.
To identify and promote best practice shared care models.
To identify and develop joint service improvement opportunities between Wide Bay Health Service District and GP Links Wide Bay.
