GP – Hospital Liaison Program (GPHLP): Fraser Coast
Program aims:
1. To undertake strategic development of systems and processes to improve patient care particularly in relation to the transition of patients between primary, secondary and tertiary care settings.
2. To bring a GP perspective to policy and planning for the provision of primary care in the Fraser Coast Health District.

Program governance:
Program governance is provided by the GPHLP Advisory Group that meets quarterly or as required. The purpose of the advisory group is to provide advice and support to the GP-Hospital Liaison Program on all matters relating to the program’s aims. The members of the advisory group are champions for the program in Hervey Bay and Maryborough Hospitals and in the local general practice community. The membership of the advisory group has been drawn from Hervey Bay and Maryborough Hospitals clinical staff, the local GP community, and GP Links Wide Bay staff.

Key objectives:
To achieve the aims of the program a number of key objectives have been set:
1. Promote the GP-Hospital Liaison Program to the GP community.
2. Liaise with TACC Team.
3. Liaise with Heart Failure Team.
4. Establish GP participation in Harvey Bay Hospital Committees.
5. Engage the GP community into Hervey Bay and Maryborough Hospitals.
6. Actively seek collaborative opportunities for service improvement between the GP community and Hervey Bay and Maryborough Hospitals.
Promote the GP-Hospital Liaison Program to the GP community
Increase awareness of the program and its objectives in the GP community. This is an ongoing part of the program that involves communicating directly to the general practice community through the GP Links website, mail-outs, meetings and promotional flyers.
Liaise with TACC Team
Liaise with the TACC Team to build and maintain engagement and to identify problems, offer advice and action outcomes. GP referrals to the TACC Team are being encouraged by the program and have resulted in a reduction in avoidable hospital admissions.
Liaise with Heart Failure Team
Identify and implement actions needed to improve engagement between GPs and the Heart Failure.
Establish GP participation in Harvey Bay Hospital Committees
Engage with Fraser Coast Health Service to increase participation in hospital committees. Regular attendance at three monthly hospital committee meetings allows GP input into hospital planning and decision making.
Engage the GP community into Fraser Coast Health Service
Increase GP participation and engagement by positive involvement in hospital activity and to develop communication links between the GP community and the acute sector. The program has created a hospitals service directory for use by the GP community. This provides an up-to-date and comprehensive service directory that is easily accessible.
Close cooperation with Fraser Coast Health Service has been undertaken to reduce avoidable hospital presentations to the paediatric department by a process of pathway mapping and collaboration between the GP community and the hospitals.
Actively seek collaborative opportunities for service improvement between the GP community and Hervey Bay and Maryborough Hospitals
Engage with both Hervey Bay and Maryborough Hospitals and the GP community to identify cooperative opportunities for improving information flow between the two sectors.
