Fraser Coast Palliative Care Referral Pathway:

PallNet: Coordinating palliative care services and education for patients and carers on the Fraser Coast
PallNet is a network of coordinated services, practitioners and community members on the Fraser Coast who respond to the needs of an informed community in promoting excellent palliative care and support to those at the end of life and their families.
PallNet has an important contribution to make in the development of community palliative care services across the Fraser Coast
Aims
PallNet will provide appropriate support, education and training opportunities / resources to service providers.
PallNet will promote community awareness of, and access to, quality care and support at the end of life. It will do this by identifying and communicating existing palliative care information, resources and activities using various strategies.
PallNet Project Manager
The PallNet Project Manager role was identified as the locally preferred way of allocating funds available from Queensland Health for the development and promotion of palliative care services on the Fraser Coast. This role is an outcome of the Blue Care Fraser Coast Palliative Care Project (2006 – 2009).
GP Links has employed the services of a PallNet Project Manager on the Fraser Coast to further develop palliative and end of life care, the palliative approach in residential aged care facilities, and facilitate education and training for primary palliative care generalist clinicians, such as General Practitioners and community based nurses.
In partnership with the recently established PallNet palliative care network, this role represents an innovative model for collaboration between stakeholders in the provision of palliative care and end of life care on the Fraser Coast.
The PallNet Project Manager contributes to both the provision of best practice in the clinical setting and the promotion of community awareness of, and capacity in responding to, issues surrounding death, dying, grief and loss.
The role of the PallNet Project Manager is one that encompasses that of service improvement through educational and professional development strategies. The PallNet Project Manager possesses leadership and mentoring skills, with a strong history as a clinician in palliative care.
The PallNet Project Manager operates across the Fraser Coast, incorporating Maryborough and Hervey Bay and their environs to:
• Provide education and professional development opportunities for primary and secondary care providers of palliative care / end of life care on the Fraser Coast
• Promote community capacity in issues surround death, dying, grief and loss
• Enhance palliative care services across all diagnoses and continuum of care settings
